Please plan to assist us with enrollment/registration by providing us with a:

1. State certified birth certificate (not hospital issued)

2. Two (2) proofs of your residency

3. Immunization record

Enclosed are the forms needed to have your child enrolled in Fremont Public Schools. These forms must be completed/signed before your child may attend Fremont Public Schools.

1. Student Enrollment / Registration Form (Grey Form)

Please complete both sides of this form, fill in every blank and answer each question as completely as possible and please remember to sign at bottom of Page 2.

2. Request for Student Record Form (White Form)

Please complete and sign this form, listing name and address of your child’s previous school.

3. Emergency Contact & Medical Information Form (Yellow Form)

Please complete both sides of this form, fill in every blank and answer each

question as completely as possible and please remember o sign at bottom of page 2.

4. Fremont Public Schools Technology Rights and Responsibilities and

Skyward “Parent Access” Application Forms (Pink Forms)

Please fill out as completely as possible and remember to sign at bottom.

5. Free & Reduced Price School Meals Family Application (Blue Form)

Please complete this form to find out if you qualify for Free/Reduced Meals for your child / children.

6. Bus Transportation Information Form (Orange Form)

Only complete this form if transportation is needed.

Further forms may need to be completed at each individual school.
