Thank you for your interest in applying to Daisy Brook Elementary. 

Please plan to assist us with enrollment/registration by providing us with:

1. Original state-certified birth certificate
2. Two (2) proofs of your residency
3. Updated Immunization record
4. Enrollment packet filled out completely

Enrollment packets may be picked up at the Dasiy Brook Office during the school year and the Administration Office during the summer.
